Should exercises be painful or not? Effects on clinical and experimental pain in individuals with shoulder pain

Shoulder pain is the third most common musculoskeletal disorder29 with the rotator cuff tendons and associated structures around the sub-acromial space most commonly involved, also known as rotator cuff-related shoulder pain (RCRSP).6 Exercise can reduce pain and improve functional disability in RCRSP,27 although the influence of exercise prescription parameters are not fully understood.28 In particular, there is a paucity of research regarding the acute pain modulatory response to exercise in individuals with chronic pain conditions.
