Resting Functional Connectivity of the Periaqueductal Gray is Associated with Normal Inhibition and Pathological Facilitation in Conditioned Pain Modulation

• Conditioned pain modulation (CPM) facilitation observed in fibromyalgia patients• Smaller periaqueducal gray (PAG) volume observed in fibromyalgia patients• PAG resting connectivity with cortical areas associated with pain inhibition• PAG-to-brainstem connectivity helps explain CPM facilitation in fibromyalgia• Consistent with pathological pain-facilitative brainstem mechanisms found in animals
