A Functional Neuroimaging Study of Expectancy Effects on Pain Response in Patients with Knee Osteoarthritis

• We used fMRI to explore the effect of acupuncture in older knee OA patients.• Pain decreased after both real and sham acupuncture when patients expected relief.• fMRI activity associated with pain relief differed for real and sham acupuncture.• More activation was found in DLPFC, OPFC and insula/putamen after real acupuncture.• More activation was found in the sACC and OPFC after sham acupuncture.• Expectancy may work by distinct mechanisms when coupled with different treatments.
