Brain mechanisms of anticipated painful movements and their modulation by Manual Therapy in chronic Low Back Pain

Heightened anticipation and fear of movement-related pain has been linked to detrimental fear-avoidance behavior in chronic Low-Back Pain (cLBP). Spinal Manipulative Therapy (SMT) has been proposed to work partly by exposing patients to non-harmful but forceful mobilization of the painful joint, thereby disrupting the relationship between pain anticipation, fear, and movement. Here, we investigated the brain processes underpinning pain anticipation and fear of movement in cLBP, and their modulation by SMT, using functional Magnetic Resonance Imaging.

Low back pain is a highly prevalent condition, causing considerable disability and burden globally19. Up to 70% of people will experience low back pain during their lifetime54. Patients with low back-related leg pain, such as sciatica, experience intense radiating leg pain that may be accompanied by neurological signs20 and both low back pain and sciatica are associated with high healthcare costs, work absenteeism and economic burden19,53.

Establishing a Research Agenda on Mobile Health Technologies and Later-Life Pain Using an Evidence-Based Consensus Workshop Approach

The health of aging populations in developed countries has not kept pace with improvements in life expectancy.9 Many older adults spend their final years coping with and managing multiple chronic conditions, many of which have pain as a primary symptom.17,29 Later-life pain is associated with substantial disability and suffering.17,29 Given the rapid increase in the number of aging adults living with chronic conditions and pain, rising healthcare costs, and shortage of caregivers to help care for affected individuals, experts have asked whether emerging technologies, in particular mobile health (mHealth) devices, can help to address these important and related challenges.

Nociceptive and Cognitive Changes in a Murine Model of Polytrauma

With a global incidence estimated to be about 106 per 100,000 population, TBI is a leading cause of trauma-related disability 15. In the US more than 1.7 million injuries occur per year resulting in 250,000 hospitalizations and 52,000 deaths 9. The majority (75-80%) of all TBI cases are, however, mild and are accompanied by the rapid resolution of the immediate symptoms including disorientation, dizziness, tinnitus, nausea and balance problems 24. The incidence of TBI is higher overall in males than females, and is particularly common in the very young, adolescents and elderly 3.

Psychological factors associated with ultra-marathon runners’ supra-normal pain tolerance: a pilot study

There is increasing evidence that athletes have higher pain tolerance than the general population.15,22,39,42,43,50 Supra-normal pain tolerance has been observed in a wide range of athletes, including marathon runners,22 triathletes,15 swimmers,43 rowers,39 ballet dancers,50 basketball players56 and contact sports athletes.42 A recent meta-analysis pooled the results of 12 studies investigating athletes’ pain tolerance and concluded that athletes had significantly higher pain tolerance than normally active controls.

Persistent Post-Mastectomy Pain: Current Status and Future Directions

Breast cancer is the most commonly diagnosed form of cancer among women in the U.S., affecting over 250,000 patients per year42. While the incidence of breast cancer diagnoses continues to climb, advances in treatment have led to decreased mortality rates, as well as increased five-year survival rates (90.8%, 95% CI = 90.5% to 91.1%). The increase in survivorship underscores the importance of attending to and optimizing quality of life for patients whose lives are extended by treatment. For female breast cancer survivors, persistent post-mastectomy/lumpectomy pain has been recognized as negatively impacting the quality of life of many (66; Peuckmann et al., 2008).
